First Year High School Confirmation,
documents & information to gather
As part of the first year enrollment process, you will need to bring the
original copies of certain documents to the office. We will make a copy of them
then and there, and return them to you right away. You will also need certain
pieces of information on hand:
•
Baptismal certificate of the student to be confirmed.
•
First Holy Communion certificate of the student to be confirmed.
•
Contact information of legal guardians, driver permissions, emergency
contact(s).
•
Information of the student: legal name, date of birth, special health
conditions, medications, allergies, sacraments already received.
